This series was converted from accessions AP34/1, AP201/3, and AP306/2. It contains forms 47A (medical examinations) and Form 41- Landing and re-entry permits for migrants (assisted and otherwise). Forms 47A were handled in South Australia from August 1951. Previously they were despatched direct from the port of landing to Immigration Central Office in Canberra.
The records are bundled together by ship name and date order. The Form 47A contains the following details: name, address, several questions relating to general health of the applicant, signature, results of medical examination including heart, blood, lungs, nervous system, mental condition, digestive organs, skeleton, skin, hearing, sight etc. The form is signed by a qualified doctor. The form also contains a black and white photograph of the applicant in the top left hand corner. On the reverse side of form there are details of children accompanying their parents and a black and white photograph.
Attached to the Form 47A is a Form 41- Landing Permit. This certifies that authority has been granted for the applicant to be admitted into Australia. It displays an official stamp of the Department of Immigration and the signiture of the Minister for Immigration. Other details on this form include: name, age, nationality, height, colour of eyes, colour of hair and special peculiarities. The form is stamped with the ships name, port of landing and the date. There is a black and white photograph of the applicant attached and some applications include extracts from marriage certificates, doctors' letters and other relevant documentation.
