How to purchase

  • Print out the (PDF 53K) and complete the details. Ensure that written information is clearly legible to avoid the wrong record being digitised and fill in as much detail as possible, particularly for your contact details.
  • Pre-payment is required in Australian dollars. Post the form with payment to the address at the top of the form, or fax it with your credit card details.
